Requesting Your Patient Records​

To request a copy of your patient records or to have your records sent to another person or healthcare provider, please contact Uprise Chiropractic through  [email protected]

Under Texas Administrative Code Rule §76.2:

• A patient may request that their patient records be disclosed to the patient or to another person.
• Requests for disclosure of patient records should be made in writing.
• The written request should include:
  
  • The specific information or records being requested.
  • The person or entity to whom the records should be disclosed.
• The request must be signed by the patient or another person legally authorized to act on the patient’s behalf.
• An oral request may be honored when appropriate identification and the required information are properly documented.
• After receiving the request and any applicable fees, patient records will generally be provided within 15 business days, unless the request may legally be denied under applicable rules.​
